How to Choose a Pediatrician
A dad's complete guide to finding, interviewing, and picking the right doctor for your baby — before they arrive.
Why this is your job
Choosing a pediatrician is one of the few pregnancy tasks that is genuinely yours to own. Your wife is already managing a full-time job of growing a human being. Researching pediatricians, scheduling interviews, and narrowing the list is a concrete, high-impact contribution you can make right now. It also means you will have a direct relationship with the doctor who will see your child dozens of times over the next few years, which matters more than most people realize before it happens.
The pediatrician you choose will be the person you call at midnight when the baby has a fever, the person who tells you whether that rash is nothing or something, and the person who tracks your kid's development from day one. Getting this right is worth the effort.
When to start looking
Start your research around Week 9 or Week 10. This sounds early, but there is a good reason: popular practices fill up, and you want enough time to gather names, check insurance, and schedule interviews without rushing. Use weeks 9 through 19 to build your short list — ask around, read reviews, and check network coverage.
Between Week 20 and Week 22, schedule and attend your meet-and-greet interviews. Two or three visits is plenty. By Week 26, you should have a final decision made and the practice notified. The hospital will ask for the pediatrician's name when you arrive for delivery, and you do not want to be figuring that out while timing contractions.
If you are already past the first trimester and have not started, that is fine. You can compress this process into a couple of weeks. The important thing is to finish before the due date, not to start at any particular week.
How to find candidates
You do not need to evaluate a hundred options. Three to five solid candidates is plenty. Here is where to start:
- Ask your OB or midwife. They refer to pediatricians constantly and know who communicates well, who returns calls, and who parents complain about. This is your single best source.
- Check your insurance network. Log into your insurer's provider directory and filter for pediatricians within a reasonable drive. Out-of-network pediatric care is expensive and unnecessary in most areas.
- Ask friends and neighbors with young kids. Parents who have been through the newborn phase recently can tell you things that no website review can — like how long you wait past your appointment time and whether the nurses are patient on the phone.
- Local parenting groups. Facebook groups, Reddit threads, and Nextdoor posts for your area often have recent, specific recommendations. Search before you post — this question gets asked constantly.
- Hospital affiliation. If you know which hospital you will deliver at, check which pediatric practices have privileges there. The pediatrician who examines your baby in the hospital is ideally the same one you will see afterward.
The meet-and-greet visit
Most pediatric practices offer free prenatal consultations, sometimes called meet-and-greets or interview visits. Call the office, tell them you are expecting, and ask to schedule one. Some things to know going in:
- These visits are usually 15 to 20 minutes. Come with your questions written down so you do not forget anything in the moment.
- Both of you should attend if possible. You are evaluating chemistry and communication style, and both of you need to feel comfortable with this person.
- You will not typically meet every doctor in the practice during the interview. Ask how many providers are in the group and how on-call rotations work.
- Pay attention to the office itself — the waiting room, the front desk staff, the parking situation. You will be here a lot, sometimes with a screaming infant. Logistics matter.
- Observe how the staff interacts with families in the waiting room. Are they warm and patient, or are they rushing people through? The front desk sets the tone for every visit.
After the visit, write down your impressions while they are fresh. Did the doctor make eye contact? Did they answer your questions without rushing? Did they seem genuinely interested in you as future patients? These gut reactions matter more than credentials on a wall.
Questions to ask at the interview
You do not need to ask every single one of these. Pick the ones that matter most to you and your wife, and leave room for the conversation to flow naturally. The point is to see how the doctor communicates, not to run through a checklist.
Practice logistics
- 1What are your office hours, and do you offer weekend or evening appointments?
- 2How does your after-hours and on-call system work? Will I reach a nurse line, an answering service, or a doctor directly?
- 3Do you separate sick visits from well visits in the schedule or waiting area?
- 4What is your average wait time past the scheduled appointment?
- 5Do you have same-day sick appointments available?
Medical philosophy
- 6What is your vaccination philosophy? Do you follow the AAP/CDC schedule?
- 7How do you handle parents who want to modify or delay the vaccination schedule?
- 8What is your philosophy on antibiotics — do you tend to prescribe them quickly or take a wait-and-see approach?
- 9What is your approach to breastfeeding support? Do you have a lactation consultant on staff or a referral?
- 10How do you handle developmental concerns or milestones that seem delayed?
Accessibility and communication
- 11How do you prefer to communicate between visits — patient portal, phone, email?
- 12What is your policy on telehealth or virtual appointments for minor issues?
- 13How many providers are in your practice, and will we always see the same doctor?
Emergencies
- 14At what point do you recommend bringing a newborn to the ER versus calling your office?
- 15Which hospital are you affiliated with, and will you examine our baby after delivery?
- 16If we call after hours with a concern, what is the typical response time?
Insurance and background
- 17What insurance plans do you accept, and what is the cost for a typical well-child visit if we have a high-deductible plan?
- 18Are you accepting new patients, and is there a waitlist?
- 19How long have you been in practice, and what drew you to pediatrics?
Red flags to watch for
No practice is perfect, but some things should give you real pause:
- The doctor seems rushed or dismissive of your questions during the interview. If they do not have time for you now, they will not have time for you at 11 PM when your baby has a 102-degree fever.
- The office staff is consistently rude or disorganized. You will interact with the front desk more than the doctor. Their attitude matters.
- The practice cannot clearly explain how after-hours care works. Vague answers like "just go to the ER" suggest they have not invested in a real system.
- They pressure you into decisions without explaining the reasoning. A good pediatrician educates; they do not dictate.
- Long wait times are treated as normal and unavoidable. Some wait is expected, but a practice that routinely runs 45 minutes behind has a scheduling problem they are not motivated to fix.
- They do not accept your insurance, and the out-of-pocket cost is unclear. Get a straight answer on cost before you commit.
- Outdated medical practices or recommendations that contradict current AAP guidelines. If a doctor is giving advice that was standard twenty years ago but has since been revised, that is a signal they may not be keeping up.
- They dismiss your questions about their approach or philosophy as unnecessary. You are interviewing them, and they should respect that.
Green flags
These are the things that separate a good pediatrician from a great one. You may not get all of them in a single practice, but the more you see, the better:
- They listen before they talk. When you ask a question, they pause, consider it, and give a thoughtful answer instead of a rehearsed one.
- They explain things clearly and without condescension. Medical jargon has its place, but a great pediatrician translates it into language you can act on.
- They are accessible after hours through a real system — not just an answering service that tells you to go to the ER for everything.
- They follow evidence-based guidelines and can explain why they recommend what they recommend. If you ask about a specific vaccine or treatment, they point to research rather than opinion.
- The office runs on time, or close to it. A practice that respects your schedule is a practice that respects you.
- They encourage questions from both parents and take each of you seriously as caregivers.
- They have a patient portal or communication system that lets you ask non-urgent questions without waiting on hold.
- They seem genuinely interested in pediatrics — not burned out, not going through the motions, but engaged with the work of helping kids.
Solo practice vs. group practice
This is one of the bigger structural decisions you will make, and there is no universally right answer. It depends on what you value most.
Solo practice
In a solo practice, one doctor handles everything. You will always see the same person, they will know your child's full history without checking the chart, and the relationship feels more personal. The downside is coverage. When that doctor is on vacation, sick, or at a conference, you are seeing a covering physician who does not know your child. Solo practitioners also tend to have more limited office hours and may not offer same-day sick appointments as easily since there is only one provider to absorb the demand.
Group practice
A group practice has multiple pediatricians sharing a patient panel. The advantages are practical: broader office hours, better availability for same-day sick visits, and a built-in coverage system when your primary doctor is unavailable. The tradeoff is continuity. You may not see the same doctor at every visit, and it can take longer to build a deep relationship. Some group practices assign a primary pediatrician and try to schedule you with them for well visits, while sick visits go to whoever is available. Ask how their system works.
In either case, what matters most is whether the doctor listens, communicates clearly, and runs a practice you can actually get into when you need them. A brilliant solo practitioner with a two-week wait for sick appointments is less useful than a good group practice with same-day availability.
Insurance and logistics checklist
Before you commit to a practice, verify these items. Print this list or screenshot it and check each one off. It takes one phone call to the practice and one to your insurer.
- Confirm the practice is in-network with your insurance plan — get the specific provider NPI verified, not just the practice name.
- Verify they are currently accepting new patients and there is no waitlist that would push your start date past the baby's arrival.
- Confirm which hospital(s) the practice is affiliated with and whether that matches your delivery hospital.
- Check the office location and drive time from your home. You will make this trip a dozen times in the first year, sometimes with a sick infant.
- Confirm office hours — including whether they offer early morning, evening, or Saturday appointments.
- Ask about same-day sick appointment availability and how far in advance well visits need to be booked.
- Verify the cost of a standard well-child visit under your plan, including any copay or coinsurance.
- Confirm the after-hours system: who answers, what is the response time, and whether they have access to your child's records.
- Ask whether the practice uses a patient portal for messaging, appointment scheduling, and accessing records.
- Confirm their telehealth or virtual visit policy for minor issues that do not require an in-person exam.
Making the final decision
After you have done two or three interviews, sit down with your wife and compare notes. In practice, the decision usually comes down to a few factors: Did the doctor listen and communicate clearly? Is the office location and schedule workable for your life? Do they accept your insurance? Did you both feel comfortable there?
Trust your gut on the interpersonal piece. Credentials and affiliations matter, but the day-to-day experience of working with this doctor will be shaped by how well they communicate with you. If you felt heard and respected during the interview, that is a strong sign.
Once you have decided, call the practice and let them know you would like to officially register as a patient. They may have paperwork to complete before the baby arrives. Also notify the hospital (or add it to your birth plan) so the delivery team knows which pediatrician to contact after birth. Aim to have this locked in by Week 26.
After the baby is born
The hospital exam
Within 24 hours of birth, a pediatrician will examine your baby. If your chosen pediatrician has privileges at the delivery hospital, they or a colleague from their practice will perform this exam. Otherwise, a hospital-affiliated pediatrician handles it and sends records to your doctor.
The newborn exam checks heart and lung function, reflexes, hips, skin tone, and overall muscle tone. The doctor will also review hearing screen results, discuss newborn metabolic screening (the heel prick test), and talk about feeding — whether breast or formula. This is your first real interaction with pediatric care, and if you have done the work ahead of time, you will feel noticeably more prepared than the parents who are scrambling to find a doctor before discharge.
The first outpatient visit
Your pediatrician will want to see the baby within three to five days of discharge — sometimes sooner if there were any concerns at the hospital. This visit checks weight (newborns typically lose some weight in the first few days and the doctor wants to make sure they are gaining it back), jaundice levels, feeding progress, and the umbilical cord stump. It is also your first chance to ask all the questions that have accumulated since you left the hospital, and there will be many.
Establishing the relationship
The first year involves a lot of pediatrician visits — typically at one week, two weeks, one month, two months, four months, six months, nine months, and twelve months. Each visit includes a physical exam, growth measurements, developmental screening, and usually vaccines. This frequency is why choosing the right pediatrician matters so much. You will be in that office constantly, and the quality of those interactions shapes your confidence as new parents. If something feels off after the first few visits, it is completely acceptable to switch practices. Most parents do not, but knowing you can takes the pressure off feeling like this decision is irreversible. It is not. The work you did during Week 22 gives you a strong starting point, and the relationship builds from there.
